Substance Abuse Case Manager - Public Health Department Job Description

Position Title: Case Manager
Department: Public Health
Reports To: Health Department Administrator / Director of Public Health

FLSA Status: Exempt
Status:  Full-Time Position 
Salary: $36,400 to $41,600


Job Summary:

A Substance Abuse Case Manager plays a crucial role in helping individuals overcome addiction and achieve lasting recovery. They act as a central point of contact, coordinating various aspects of a client's care and providing support throughout their journey.

Key Responsibilities:

Assessment and Evaluation: Conduct thorough assessments to understand clients' substance use history, mental health, social circumstances, and individual needs. 
Treatment Planning: Collaborate with clients and other professionals to develop personalized treatment plans with specific goals and interventions.
Resource Coordination: Connect clients with essential resources such as:
     Detoxification programs
     Rehabilitation facilities
     Therapists and counselors
     Support groups
     Housing assistance
     Employment services
Legal aidMonitoring and Advocacy: Regularly monitor clients' progress, provide ongoing support and encouragement, and advocate for their needs within the healthcare and social service systems.
Crisis Intervention: Provide immediate support and intervention during crisis situations, such as relapses or mental health emergencies.
Documentation and Reporting: Maintain accurate records of client interactions, progress notes, and other relevant information.

Essential Skills and Qualifications:

A Recovery Coach is a supportive mentor for people seeking recovery from addiction or mental health challenges. They provide guidance, encouragement, and practical strategies to help individuals achieve their recovery goals and live fulfilling lives.

Job Duties and Responsibilities

Provide Support and Guidance: Offer one-on-one support, active listening, and empathy to individuals in recovery.
Develop Recovery Plans: Collaborate with clients to create personalized recovery plans with achievable goals and action steps.
Offer Resources and Referrals: Connect clients with community resources, support groups, treatment centers, and other services.
Teach Coping Mechanisms: Help clients develop healthy coping strategies for triggers, cravings, and challenging situations.
Promote Healthy Habits: Encourage clients to adopt healthy lifestyle practices, such as exercise, nutrition, and mindfulness.
