How to Renew My Annual Food Establishment Permit
In Mingo County ALL food establishments are required to have a food service permit to operate. In order to renew your annual permit for your food establishment the following information must be submitted with appropriate fees to the health department before May 15th in order for the Mingo County Health Deparment to have received by May 31st. The purpose for this, all food establisment permits are renewed on July 1 of each year and rule 64CSR17 states that all food establishment permits must be recieved 30 days prior to their expiration date of June 30th to achieve timely renewal. When do I submit, what does it cost?
Application for Food Establishment Permit renewals must be submitted and paid 30 days prior to June 30th. This is to help the facility in case adjustments must be made to meet our codes and regulations. Food Service Establishment Permit Fee are base on the seating capacity and liquor license requirements are as follows:

51-80 | $375 |
81 and over | $500 |
* If the facility has a liquor license from the West Virginia Alcoholic Beverage Control Administration – an additional $125 fee shall be added to each seating capacity amount. The fixed expiration date is June 30.

Once food establishment permit is granted the establishment must be inspected and an operational permit issued prior to opening. Food service establishments are then inspected a minimum of once each six months in accordance with the U.S. Food and Drug Administration’s (FDA) 2013 Food Code.
